APS' Medicaid Department is available to help families enroll in Medicaid or refer for other eligible service. Please call 505-855-5261 or 505-855-9820 (Spanish speakers) for more information. 

Health Screenings

Health screens are completed yearly for Pre-k, Kindergarten, 1st, 3rd, and 5th grade and for students new to APS. These screenings include vision, hearing, height/weight, and dental.

Parents/guardians will be notified if there is any need for further attention in these areas.

If you would like to opt out from having your child screened, please notify the health office in writing.

Welcome to the health office

My name is Lea Reynolds and I am the school nurse this year.  

I am here to help your students stay safe and healthy while learning, whether they are learning online at home, or in person at school.

As you know, school looks different this year due to the COVID-19 pandemic. We recognize that there is much concern and stress about how we will protect your children and ourselves during this global pandemic. I want to share some information with you about what to expect from the health office this school year.

Each day before sending your child to school, please review the following questions regarding your child’s health.

  1. Does your child currently have a temperature of over 100.4 degrees?
  2. In the past 24 hours, has your child experienced:
    • Sore throat, runny nose, congestion
    • New frequent, dry cough (for students with chronic allergies/asthma, a change in baseline cough)
    • Shortness of breath or difficulty breathing
    • Diarrhea, vomiting
    • Loss of taste or smell
    • New onset of headache
    • Fatigue or muscle or body aches
  3. Has your child recently been in close contact with anyone who has exhibited any of these symptoms?
  4. Has your child recently been in contact with anyone who has tested positive for COVID-19?

If you or your student are COVID positive and in isolation and/or quarantine, please let me or the principle know. We ask so we can contact you to support you and ensure we coordinate contactless materials distribution (delivery by hand or by mail) so you don't have to leave isolation/quarantine to come to the drive by pick-up. This is also so that I can check-in with you to support your family with health information and resources.  ​

If you answer yes to any of the questions:

Please do not send your child to school. Contact a medical provider or call the COVID Hotline at 855-600-3453, then call the health office at your child’s school.

Please let me know if your student has a chronic health condition for which s/he will need nursing support this school year.

If your student(s) will require medication or a procedure while at school, please have their provider fill out their specific form. See Health Forms. I cannot give a medication or carry out a procedure without this signed document.
